Create your first form
A form is the foundation of feedback collection. Create it from the Surveys menu, build questions on the Build tab, then use Logic, Preview, and Settings. This article covers the first steps.
What you will see
The left menu shows "Surveys" with all forms in the workspace.
Each form has tabs: Build, Logic, Preview, and Settings. Build is where you add questions.
In Build you see a preview on the left and an edit panel on the right. At the top is a button to add a new block (question or section).
The block menu shows categories: Text (Short text, Long text), Contact (Email, Phone), Numeric (NPS, Rating, Opinion scale), Choice (Single choice, Multiple choice, Yes/No), and Other (Statement, End screen).
How to set it up
1. Click Surveys in the left menu.
2. Click "New form" or "New survey" to create a blank form.
3. Name the form in the field at the top and open it (Build tab).
4. Click add block (plus or "Add") and choose a question type, e.g. Email or NPS.
5. In the right panel enter the question text and options for choice or scale questions. Save changes.
6. Add more questions if needed and use Preview to test. Then use Share for the link or embed code.
Tips
Start with a short survey (3–5 questions); you can add more later.
Use the End screen block to thank respondents; it is under Other in the block menu.
On the Logic tab you can show follow-up questions based on answers; see Set up logic.
Common issues
No "New form" button: check that you are in the correct workspace and have permission to create forms.
Changes not saved: click Save after editing or wait for auto-save if enabled.
